New Patient Forms

New patient intake forms can add 10-15 minutes of your time in our office.  If you would like to expedite this process you can show up with the listed documents below printed and filled out completely in order to move things along.

Cancellation & No-Show Policy

There is a fee of $50 if the office is not notified of your cancellation 24hrs prior to your scheduled appointment or if you decide to cancel same-day.

We understand that things come up and sometimes you need to reschedule.  We also know your time is valuable and wish to honor your scheduled meeting with our doctors - and we extend this courtesy to all of our patients.  We would greatly appreciate you letting us know if you cannot make it so we can allow another patient who is waiting for an appointment to be scheduled in your stead.  This allows us to avoid overbooking and long waits. Thank you for your help in making a better experience for everyone.

Out of Network Patients

Being “Out of Network” means that the physician you are seeing is not a preferred provider with your specific insurance plan.

 

For patients who are Out of Network we require full payment by cash, check, or credit/debit card at the time of service during checkout.  As a courtesy, our office will gladly provide you a superbill via email or fax that has the information required to make a claim with your insurance.  Reimbursements will vary depending on your insurance’s Out of Network rates.

 

Although we accept most PPO insurances, please check with your insurance company to make sure we are a provider for your plan before your appointment.  This is a patient responsibility. We do not accept any HMO plans. All providers take Medicare Part B and Medicare Advantage PPOs. Please bring your photo ID and insurance card to every visit.

Kaiser Permanente Patients

We are only accepting Kaiser Permanente patients via referral from their KP primary care doctor.  Please have your referral letter ready to provide for us at the time of your appointment. You may also have it emailed to us at: office@mosaicdermatology.com or via fax: 310-453-3104.

Former Patients of Dr. Strick & Dr. Drayton

We have your previous charts retained off-site if you were seen as a patient in the last 7 years.  By HIPAA standards we are required to have you sign new patient paperwork if you have not previously seen Dr. Madkan, Dr. Wang, or Dr. Hamill.  We also want to ensure your information is up to date in our system.
