There is a fee of $75 if the office is not notified of your cancellation 24hrs prior to your scheduled appointment or if you decide to cancel the same day. We understand that things come up and sometimes you need to reschedule. We also know your time is valuable and wish to honor your scheduled meeting with our doctors - and we extend this courtesy to all our patients. We would greatly appreciate you letting us know if you cannot make it so we can allow another patient who is waiting for an appointment to be scheduled in your stead. This allows us to avoid overbooking and long waits. Thank you for your help in making a better experience for everyone.
Insurances We Take
Although we accept most major insurances, it is a patient responsibility to check that their Mosaic Dermatology provider is in-network for their insurance plan prior to their appointment. If we bill your insurance and your Mosaic Dermatology provider is out-of-network you will be responsible for the full cost of your appointment.

Out of Network Patients
Being “Out-of-Network” means that the provider you are seeing is not a preferred provider with your specific insurance plan. For patients who are out-of-network we require full payment at the time of service. As a courtesy, our office will gladly provide you a superbill via email that has the information required to make a claim with your insurance. Reimbursements will vary depending on your insurance provider’s out-of-network rates.

New patient intake forms can add 10-15 minutes of your time in our office. If you would like to expedite this process please email or call our office and we will send you the paperwork to be filled out securely through DocuSign.
